Job Description

Care Assurance Aging Care Management & Consultation is looking for a Part-time/PRN - RN & LPN Case Managers for their Medicaid Case Management division. This position is ideal for semi-retired nurses or nurses looking to add additional income and have flexibility.

We are looking for a qualified RN & LPN Case Managers to provide case management assessments and oversight to our members and act as a key team member of our agency. Our ideal candidate has solid experience as a registered nurse or a licensed practical nurse and is able to handle stressful situations. This position requires the ability to communicate efficiently, an eye for detail, experience with conducting in-person and telephonic assessments, and a problem-solving attitude. Experience as an MDS coordinator is strongly preferred but not required.

Responsibilities

• Conduct thorough assessments (assessments are currently conducted telephonically due to COVID-19 but will resume to in-person visits in the future. )
• Review the CCSP/SOURCE Program with the member
• Ensure program requirements and objectives are met
• Communicate the status of member's with doctors and other providers involved in the member's care
• Communicate effectively with the case management agency

Skills

• Proven work experience with MDS assessments strongly preferred but not required
• In-depth knowledge of senior care, long term care, and Georgia Medicaid preferred
• Thorough knowledge of legal regulations and best practices in healthcare
• Strong clinical documentation management skills
• Must be proficient using a computer (must have access to a laptop or desktop)
• Great communication and people skills
• Excellent organizational and problem-solving skills
• Completion of a US practical nursing program is required
• A valid practical nursing license in the state of Georgia is required
• **Must live within NE Georgia & Metro Atlanta Areas

This position is remote during Covid-19 but will require in-person visits for all assessments once the public health condition is considered safe for in-person interactions with the senior population.

Assessments are paid at a flat rate. Case management conferences are paid at hourly rates. Monthly pay.
