Job Description

MINIMUM REQUIREMENTS:

  • Must be at least 21 years of age.
  • Bachelors Degree in social work, psychology, human services, or related health/human services discipline.
  • Prefer at least one-year experience in area of child/adolescent residential treatment, mental health, community based services, human services, and/or counseling
  • Must be flexible with work schedule to ensure efficient operations.
  • Must have a valid driver’s license.

PHYSICAL AND SENSORY REQUIREMENTS (with or without reasonable accommodation):

Walking; reaching; bending; pushing; pulling; lifting heavy loads (i.e., at least 50 pounds).


ESSENTIAL JOB FUNCTIONS:

  • Responsible for carrying out the following activities with assessed patients and their families as assigned by and under the supervision of the Clinical Director:
    • Psycho educational Assessment and/or Groups
    • Case Management Services
    • Community-based referrals
    • Consumer Advocacy
    • Therapist meeting and scheduling weekly and daily as needed
    • Crisis Intervention as a Paraprofessional
    • Development of Community Resources
    • Community Support Services and Support
    • Mental Health Prevention/education
  • Responsible for obtaining information needed for generation of Master Treatment Plans (i.e. Initial Treatment Plans including target dates and need for admission, psychosocial, discharge criteria).
  • Ensure and generate completed Master Treatment Plans within 14 days of admission.
  • Attend Utilization Review meetings each week and function as secretary, taking meeting minutes as needed.
  • Meet with therapists weekly as needed following the Utilization Review meeting to ensure all Treatment Plan updates are completed for the week as indicated on the submission cycle and update on the Master Treatment Plan.
  • Attend patient staffing each week with physicians as part of the Treatment Team to ensure the flow of information to and from the Utilization Review Department.
  • Attend Master Treatment Plan meetings each week for new admissions.
  • Report all pertinent information from patients, staff and Master Treatment Plan meetings to Utilization Review Nurse to be included in reports submitted to APS for re-certification of continued stay.
  • Work in partnership with Therapist and Treatment Team on an ongoing basis to ensure all information is accurate and up to date.
  • Work together with Utilization Review Director and Utilization Review Nurse on an ongoing basis to ensure all information regarding Master Treatment Plans, Master Treatment Plan reviews and discharge planning that is reported to APS is accurate and up to date.
  • Responsible for assessment of each assigned patient and their documentation.
  • Provide assistance in evaluating patient progress.
  • Provide assistance in maintaining treatment notes (i.e. Group, Family, and Individual).
  • Responsible for composing 30 day DYS and DHS reports.
  • Compose court reports/updates and provide agency representation in court as required.
  • Maintain monthly update letters to initial referral agency.
  • Maintain and ensure accuracy of all patient records.
