We are seeking a mission-focused professional to serve as a Case Coordinator responsible for providing in-home support services for Sidney, Ohio and Surrounding areas Division foster parents and children.
As a Case Coordinator you would:

  • Work independently and create your own schedule based on your caseload and availability of your assigned families. This includes potentially working until 8pm several nights a month, based on the needs of the families.
  • Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 12-15 clients that you'll see weekly.
  • Participate in our, paid rotating on-call schedule.
  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Diagnosis, Treatment Plans and Progress Notes, and Monthly reports. Overall documentation time is about 20%.
  • Develop and maintain consultative and liaison relationships with members of the child’s treatment team (biological parents, school and SAFY team members)
  • Travel to FOSTER families’ homes, in and around Sidney, - mileage reimbursement included.
  • Overall hours being worked outside of the office are estimated to be around 60%, which includes traveling to and from visits and time spent conducting home visits.

What you bring to this role:
Education

  • Bachelor’s degree in the field of social work, counseling, psychology, sociology, child and family studies, or other approved related field of human services
  • Required Licensure- LSW or LPC

Experience

  • Two years of relevant experience is preferred.
  • Experience in child welfare preferred, OR relevant experience in working with children in general.
  • Five years of relevant experience preferred with related field of human service degrees.

Skills

  • Strong organizational skills to prioritize workload
  • Excellent communication skills (oral and written)
  • Demonstrated skill of writing treatment plans, case notes, etc.
  • Operate a computer in regards to Microsoft Office and agency database system
