About the St. Josephine Bakhita Program

The St. Josephine Bakhita Program is a therapeutic home for pregnant and parenting women between the ages of 21 and 35 and their children. The women in the program have substance abuse and mental health needs, are in jeopardy of losing custody of their children, and have been referred to the Mother’s Recovery Program by the Illinois Department of Children and Family Services. This program provides a safe and secure home, mental health services, substance abuse treatment and counseling, parenting classes, vocational training, life skills education and academic services to prepare the women for a successful transition back into their communities as productive adults and to help them develop healthy relationships with their children. In addition, the program provides a children’s curriculum to address the needs of the children living with their mothers.

About the Case Coordinator Position

The Case Coordinator acts as an advocate for Maryville clients, supporting the clients in exercising their rights, choices, and responsibilities. Assists in the monitoring of program development and treatment planning in order to ensure a safe and nurturing environment. Position requires the consistent exercise of discretion and judgment.

Essential Functions


  • Provides frequent face to face contact and social services, ranging from routine to complex, in accordance with the terms agreed upon in the Individual Treatment Plan; identifies reports and recommends changes in services.
  • Completes a comprehensive assessment or reassessment of the Individual Treatment Plan of the assigned client within the time frame established by the designated agency and develops a written service plan for each assigned client.
  • Prepares court reports, attends all court hearings and carries out orders of the court as they pertain to client services.
  • Completes the client intake forms.
  • Facilitates the transfer of records and employment history, and schedules conferences with appropriate parties.
  • Provides hands on information regarding community resources. Assists the clients in the utilization of available resources by providing linkage to appropriate resources.
  • Obtains timely reports regarding the clients and provides information to appropriate authorities.
  • Conducts the initial assessment within fourteen working days of the client’s admission. Reviews initial Individual Plan with the caseworker.
  • Ensures/monitors the client’s required medical and dental needs and checkups.
  • Maintains and ensures that case files are organized and contain all necessary information.
  • Reports all unusual incidents immediately to the supervisor.
  • Helps the clients set, develop and attain realistic personal, educational, social and employment goals.
  • Organizes and schedules regular and emergency clinical staffings, ensuring notification and invitation of all required stakeholders. Documents staffings in accordance with protocols, and within required timeframes.
  • Participates in staffing, staff meetings, treatment team meetings and in-service training sessions.
  • Documents all case management activities on Case Work Records.
  • Completes compliant Medicaid billing for those case management services prescribed for the client.
  • Ensures the Quality Utilization Reviews are completed in a timely manner, and that recommendations from reviews are implemented in a timely fashion.
  • Ensures that required forms are completed and submitted within required timelines.
  • Schedules visitations for clients in accordance with their permanency goals.
  • Coordinates discharge planning for the clients from the time of intake, including the formulation, modification, and upkeep of the Comprehensive Treatment Plan.
  • Ensures the coordination and completion of the Residential Transition and Discharge Protocol, including documentation and scheduling of post-transitional services.
  • Communicates regularly with clinicians, program management, and the Program Director to ensure continuity and effectiveness of the treatment program.
  • Participates in therapeutic interactions at the discretion of the Program Director and therapist.
  • Transports clients in a Maryville van, as needed.
  • Participates in continued performance quality improvement.
  • Other duties as assigned.

Minimum Requirements

Education/Experience:

  • Bachelor’s degree in human service related field.
  • One year residential care experience.
  • Minimum of 25 years of age

Physical Demands:

  • Required to frequently sit, stand, bend, stoop and walk, sometimes for extended periods.
  • Required to lift/ move objects weighing in excess of 20 pounds.
  • Must have visual, hearing, and learning capabilities sufficient to perform the essential functions defined above.

Environment and Scheduling:

  • Duties may involve contact with clients who may be threatening, aggressive and/or under the influence of drugs or alcohol.
  • Must be flexible and available to work additional hours when necessary.
  • May be scheduled to work on weekends and/or holidays
