Purpose:
Provide personal care, household management, and companionship services as designated in the customer's plan of care. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.

Responsibilities:

  • Document care provided, within the established guidelines of the plan of care.
  • Report, document, resolve and follow up on client complaints and concerns.
  • Perform all direct customer care in accordance with established Plan of Care and within the guidelines of agency policies and procedures.
  • Participate with employee recruitment and selection.
  • Responsible for observing and immediately communicating to the Care Supervisor any changes in a customer's condition or suspected cases of abuse or neglect.
  • Assist with marketing and promotion of services.
  • Provide hands on practical training in the field to new employees that shadow them during orientation in order to enhance their personal care skill performance and approach to providing excellent customer service.
  • Assist with orientation and ongoing staff training as required by the DOH regulations.
  • Perform duties of Care Giver including client assistance with: medication reminders, personal care, household management tasks, laundry, shopping and errands, pet care, meal preparation, companionship, escorting, and specialized tasks.
  • Help to ensure client documentation is compliant with PA Department of Health regulations.
  • Maintain and protect customer, staff, and Agency confidentiality.
  • Function as a mentor to newly hired staff during orientation and ongoing as needs arise.
  • Coordinate caregiving schedules for staff.
  • Submit documentation within Agency defined timeframes.
  • Intake and assign customers/clients to direct care staff.
  • Participate in on-call rotation.
  • Regularly attend required staff meetings.

Qualifications

  • High School Diploma or GED preferred.
  • 1 year of related work experience and CNA required or 2 years of related work experience required.
  • Access to transportation in the Agency service area with valid Pennsylvania driver's license and auto liability insurance.
  • No weight restrictions for lifting or moving.
