Job Description
Responsible for all areas of Operations; human resources, intakes, clients, caregivers and scheduling

Administrative
  • The Client Care Coordinator shall report any signs of abuse, neglect, mistreatment, misappropriation, or exploitation immediately to the appropriate legal division and to the Agency Administrator
  • The Client Care Coordinator is responsible for conducting new caregiver/client introductions.
  • The Client Care Coordinator is responsible for performing Quarterly Monitoring Visits and will ensure that all appropriate paperwork is turned in to Agency Administrator in a timely manner per agency policy.
  • The Client Care Coordinator is responsible for updating the clients daily care plan as needed. The CCC shall make a copy of updated daily care plan and put in clients file in the office. CCC to notify Agency Administrator of any changes.
  • The Client Care Coordinator is the primary point of contact for clients and caregivers to answer questions and facilitate problem resolution.
  • The Client Care Coordinator provides caregiver skills training each week during new employee orientation, and ongoing caregiver training in the field as needed.
  • The Client Care Coordinator shall be knowledgeable of caregiver’s skills, note any deficiencies, and schedule additional training in office or in the field as necessary.
  • The Client Care Coordinator will take part in the monthly on call/weekend rotation.
    • On call 24/7.
    • Phone Line responsibility is from 4:00 pm- 8:00 am Monday-Friday. 24/7 Phone Line responsibility every other weekend.
  • The Client Care Coordinator will be on call when Administration is out of office; i.e. during annual conferences and/or regional conferences throughout the year. Provide documents for admission and/or supervisory visits to RN, as appropriate.
  • The Client Care Coordinator shall assist them In Home Care Agency Administrator with office administration tasks. This includes but is not limited to the following: Entering client and caregiver information, service/care plan, notes, etc. into AxisCare, assist with scheduling assessments and revisits, follow up with healthcare providers, Case Managers and clients regarding care plan, and other tasks as directed.
  • The Client Care Coordinator shall fill-in for caregivers as needed when caregivers call off last minute, for caregiver no-shows, or if/when shifts cannot be filled.
  • The Client Care Coordinator will follow-up on referrals/inquiries of prospective clients for possible admission.
  • The Client Care Coordinator will create caregiver folders both in the computer and a physical file – making sure that all documentation is current.
  • The Client Care Coordinator will supervise caregivers and complete performance appraisals for caregivers at specified intervals.
  • The Client Care Coordinator will act as the Agency’s ongoing liaison through marketing events.
  • The Client Care Coordinator will implement Agency’s policies and procedure and ensure Agency adheres to all State and Federal guidelines.
  • The Client Care Coordinator will call clients on a monthly basis to enquire about quality of service; attend community meetings, and send out in-service exercises to caregivers on a monthly basis.
  • The Client Care Coordinator will determine staffing need.
  • The Client Care Coordinator will audit client and caregiver files at least every 4-months to ensure compliance.
  • The Client Care Coordinator will answer telephone, take inquiries or messages and address issues using excellent telephone etiquette.
  • The Client Care Coordinator will respond promptly and courteously to all calls.
  • The Client Care Coordinator will protect the confidentiality of clients, caregiver, and agency

Scheduling
  • Create, input and maintain staff schedules in AxisCare.
  • Ensure that the numbers and qualifications of personnel available to provide services are sufficient to meet the needs of the clients
  • Triage incoming calls from caregivers/clients for scheduling changes or services needed
  • Schedule client appointments/visits according to care plans and staff availability
  • Contact care providers and clients regarding day-to-day changes
  • Discern client services required as outlined in agreements, urgent requests and care plans
  • Enters staff and client information into database
  • Maintain staff and client database (e.g. CQA, dates and reminders of TB, client admission, supervisory visits, client hospital admission and discharge).

Qualifications/Educational Requirements
  • Ability to multi-task and work well in a fast-paced environment while ensuring attention to detail
  • Must possess a current driver’s license, proof of vehicle registration and valid insurance
  • Must possess the ability to communicate effectively and interact with clients, client representatives, family members, coworkers, and administrative staff
  • Must adhere to Agency policy and procedures at all times, and notify Agency Administrator immediately with knowledge of deficiency
  • Must be certified in CPR and First Aide
  • Excellent interpersonal skills
  • Excellent organizational abilities
  • Excellent customer service skills and the ability to ensure all client and employee needs are met in a timely manner
  • Proficiency with Microsoft Windows based programs such as Word, Excel, Outlook, HomeTrak and internet based applications, etc.
  • Able to work independently, demonstrating common sense and sound judgment, personable
  • and a team-player
  • Ability to participate in after-hours on-call rotation
  • Ability to pass background check, including criminal check
