Job Description

$750 signing bonus for new hires (paid out at $250 @30, 60 and 90 days)

Schedule is: Mon-Fri 9am-3pm

Starting pay $16 an additional $1 weekend differential. Medical insurance starting at $25 biweekly.


**This position requires the use of a personal vehicle and proof of car insurance


ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Maintain respectful communication and interactions with consumers at all times.
  • Ensure that individuals have resources needed to make informed choices and are active participants in decisions affecting their lives and supports received.
  • Assist individuals with personal care needs and skill training.
  • Follow all diet protocols as ordered by a physician
  • Report any suspicions of abuse or neglect to the Disabled Persons Protection Commission as a mandated reporter.
  • Report any injuries or potential health problems to program nurse.
  • Assist all individuals with their daily needs and ensure that all activities are reflective of their choices.
  • Take initiative to integrate individual into the community in areas of his/her choice.
  • Assist all individuals in meeting goals identified in all areas of the ISP/DHSP and complete documentation as indicated.
  • Expose individual to new ideas and choices, which assist in their continued growth and development.
  • Is responsible for the safety and well being of all individuals. Is aware of and follows all Individual and Program Specific Safety Plans and approved behavior programs.
  • Ability to carefully monitor and supervise individuals served.
  • Is prepared for and actively participates in all mandatory meetings.
  • Demonstrate flexibility in meeting program needs.
  • Respects the role and responsibility of his/her supervisor and maintains the same work diligence whether his/her supervisor is present or not.
  • Maintain a courteous professional attitude while conducting business by effectively communicating with all LifeStream personnel, family members, and all community contacts.
  • Completes all assigned tasks in a timely manner with minimal assistance.
  • Maintain cleanliness of program areas
  • Avoids bringing personal matters to work that could have a disruptive impact on the consumers or co-workers
  • Consistently advocates for the individuals served in all areas above.
  • Must maintain current NVMAB certification and attend all mandated behavioral trainings

QUALIFICATIONS AND REQUIREMENTS:

  • Maintenance of an acceptable CORI
  • Valid Driver’s license
  • Negative drug screen
  • Use of personal vehicle for work purposes
  • Commitment to the philosophy of community inclusion

  • EDUCATION and/or EXPERIENCE:
    • High School Diploma or GED
    • One year experience in the field of Human Services
  • Demonstrated history of quality work performance and teamwork in previous positions

  • OTHER SKILLS and ABILITIES:
  • Proficient in reading and writing English and the ability to perform basic math skills. Basic computer skills.
