Job Description

Merakey is seeking a Blended Case Manager (Residential setting) to join our Behavioral Health team, in Philadelphia County, PA.

POSITION OVERVIEW

The Blended Case Manager performs diversified case management duties for a complex caseload consisting if clients with multiple diagnosis and psycho-social problems including criminal charges.

These duties include assessments, group and individual treatment, discharge planning and related duties.

The Blended Case Manager provides testimony to the courts as required, communicates with jail mental health staff regarding concerns about or special needs of defendants, and arranges for placement of committed individuals.

The Blended Case Manager consults with judges, attorneys, and probation officers regarding mental health issues and the disposition of mentally ill defendants.

The Blended Case Manager coordinates with Crisis Services when acute care is needed, coordinates with community providers regarding referrals for services for forensic clients, and provides training on mental health issues.

This position will report to the Program Director.

Requirements:

Bachelor's degree in a human services field

Valid Driver's License.

Minimum 2 years of mental health experience in a group home or residential setting; forensic knowledge preferred.
