Job Description
Overview: Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking a full-time Behavioral Health Program Coordinator for our Bloomington, IL facility.

Responsibilities:
  • Manage Chestnut Family Health Center behavioral health services including but not limited to mental health prescriber services, clinical services, case management and community support.
  • Manage the development, planning, implementation and evaluation of behavioral health services according to all governing policies and procedures to improve services.
  • Provide consistent leadership direction and oversight of CFHC’s integrated service delivery approach.
  • Develop for review/approval policies and procedures for all oversight areas including but not limited to behavioral health, MAT, case management and community support.
  • Set an example for and reinforce evidence-based practices, including but not limited to, the recovery philosophy utilizing the elements of hope, respect, empowerment, health/wellness and spirituality/connectedness and harm reduction.
  • Act as a role model and provide leadership and direction to ensure positive interactions within Chestnut and the community.
  • Develop, maintain, implement and oversee assigned areas of staff training plan incorporating use of on-line learning system. Train and evaluate assigned staff and interns.
  • Provide supervision to assigned staff in a manner determined as appropriate per the level of staff licensure, degree or position.
  • Complete onboarding HR paperwork, competency checklist and staff evaluations within established timeframes.
  • Manage CFHC Medication Assisted Treatment (MAT) services at assigned locations.
  • Serve as project lead on assigned grants or projects.
  • Develop and maintain assigned policies and procedures.
  • Actively participate and/or take a lead role when appropriate in assigned committees or meetings including but not limited to management team meetings, quality management and safety committee meetings, Leadership Council meetings and co-occurring disorders task force meetings.
  • Oversee the development and maintenance of community resource referral networks for each CFHC site and serve as liaison between CFHC and external community resource vendors.
  • Work with CFHC management and QI Manager to develop and monitor outcomes in relation to impact of social determinants of health and behavioral health interventions.
  • Promote Chestnut’s culture of customer service excellence through the adherence to the seven behavioral standards for customer service.
  • Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and consumer information.
Qualifications:
  • Must have a master’s degree in a human services field and qualify as a Licensed Practitioner of the Healing Arts (LPHA) in the State of Illinois.
  • Three (3) years’ clinical experience required.
  • Illinois licensure of LCSW or LCPC also required.
  • Two (2) years’ supervisory experience preferred.
  • Must have a valid driver’s license, private automobile insurance and be insurable.
  • Satisfactory completion of Chestnut’s background checking process.
