The Rehabilitation Assistant is a key associate at Hope Network/New Passages whose primary responsibilities include: To provide medically necessary and appropriate personal care and community living supports to persons with intellectual developmental disabilities, persons with severe mental illness, and/or individuals with co-occurring diagnosis. The Rehabilitation Assistant provides suitable services with respect and dignity to all persons-served. The Rehabilitation Assistant has a high degree of personal integrity, generates positive energy, and makes a difference every day.
Summary:
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Regular and predictable attendance is an essential requirement of this position.
2. Provide supreme customer service, always being courteous, responsive, professional, and helpful to the organization’s internal and external customers.
3. Know the contract requirements for program/service.
4. Participate in Person-Centered Planning meetings, as requested.
a. Make recommendations concerning treatment planning as appropriate.
b. Assist with the implementation of the persons served, Individualized Plans of Service and make recommendations concerning treatment planning to the Line Manager, as needed.
5. Provide community living supports to include, but not limited to: meal preparation, laundry, routine household care and maintenance, activities of daily living such as bathing, dressing, personal hygiene and shopping, money management, non-medical care activities, socialization and relationship building, leisure and community activities, and attendance at medical appointments, etc.
6. Provide, as appropriate, personal care services to include, but not limited to: eating/feeding, toileting, transferring, etc.
7. Dispense, administer, and monitor medication as indicated, via professional order.
a. Document dispensing and/or administration of medication immediately after the event.
8. Engage persons served and monitor their wellbeing. For persons served living in specialized residential settings, engagement includes monitoring their well-being every 15 minutes and document activity immediately following the event.
9. Engage persons served in problem solving, skill training, and side-by-side activities.
10. Lead or participate in the Family/Guardian/Friends Liaison Program activities, building a friendly and open communication network with the natural supports of persons served.
11. Create in others an awareness of their abilities and self-worth.
12. Document on the progress, or lack of progress, towards goals of persons-served at the close of each shift.
13. Assist with facility and vehicle maintenance, which may include indoor and outdoor tasks.
14. Provide and/or assist with transportation related to personal care, community living supports, employment and/or planned activities.
15. Actively monitor and provide feedback on defined line items in budget, as directed by manager.
16. Ensure 100% of the contract/grant compliance reporting requirements are completed and submitted by due date.
17. Adhere to high level of health and safety practices in physical facilities and with corporate vehicles.
18. Adhere to applicable licensing rules, accreditations standards, Code of Ethics and New Passages Policies and Procedures, including Corporate Compliance Plan and Personnel handbook.

Educational / Talent Requirements:
1. High School diploma or GED, preferred.
2. Ability to lift up to 25 pounds.
3. Ability to write and/or manipulate a computer keyboard.
4. Ability to hear and speak clearly.
Work Experience Requirements:
1. Valid motor vehicle operator license, preferred (Driver’s License may be required by some programs).
