Job Description

Director of Nursing / Supervisor Report to the administrator.

BBHC has a registered nurse who currently has unrestricted licensed by the Virginia State Board of Nursing to supervise the nursing visits & personal care services.

The duties of the registered nurse supervisor shall include:

• Assuring that the initial in-home evaluation of any patient requiring nursing and personal care.

services of a nurse aide, home health aide, or other nursing support staff are profiled by a registered nurse.

• Initiating the patient’s plan of care following nursing needs specified in the approved plan of treatment.
• Reevaluating the patient’s nursing needs every 60 days.
• Delivering or supervising the delivery of skilled nursing services if applicable
• Initiating preventive and rehabilitation nursing procedures.
• Preparing and coordinating clinical notes and progress notes on nursing services delivered.
• Assigning of registered nurses, licensed practical nurses, home health aides, and other nursing support staff to individual patients.
• In-home supervision of nursing services delivered by all licensed personnel, certified nurse aides, home health aide, and other nursing support staff.

a.If BBHC contracts with other agencies or legal entities to deliver registered nurse, licensed practical nurse, certified nurse aides, home health aide, or nursing support services.BBHC nursing supervisor shall be responsible for assuring that the in-home supervisory visits are conducted by the contractor.

b.In-home supervisory visits shall be made when the licensed personnel, CNA, home health aide, or other nursing support employee being supervised is present and is delivering services to the patient.

• Reporting changes in the patient’s medical or mental condition to the attending physician and CAN employees and contractors involved in the patient’s care.
• Teaching or supervising the teaching of household members who are responsible for assisting the patient with his/her nursing or personal care needs.
• Conducting orientation and training or supervising the orientation and training of agency employees assigned to the nursing service if needed.
• Participating in the quality assurance reviews and evaluations of the nursing service. And infection control team
