Outreach is seeking an experienced and extremely motivated PTime Nurse Practitioner to work in our Outpatient Opioid Program located in Brentwood, Long island.


What You Will Do:
  • Perform comprehensive and focused physical exams, assessments, perform, supervise, order tests, interpret diagnostics and laboratory tests, make diagnoses, initiate and manage treatment including prescribing medication and non-pharmacologic treatments, coordinating care, counseling, and educating clients and their families and communities.
  • Follow up on any health-related referrals made.
  • Maintain a tracking system to monitor patient health issues, medical appointments, and prescribed medications (internal/external).
  • Provide patient education and counseling around health issues.
  • Provide staff education around health-related issues.
  • Provide crisis intervention on an emergency basis when indicated.
  • Complete all needed documentation and reports in a timely manner. Inform Program leadership (Director, Asst. Director, and/or Coordinator) of any problems or anticipated inability to meet deadlines.
  • Actively participate in staff meetings, case conferences and other unit meetings.
  • Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.
  • Conduct toxicology testing, as needed.
  • Identify the current health needs of the patient population and implement activities to address service and informational deficits.
  • Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
  • Insure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.
  • Is a designee for the Medical Director to sign and authorize urine and oral fluid testing on laboratory test request forms.
  • Interact with staff, clients, students and visitors in a courteous, professional and respectful manner that is consistent with the agency’s mission and values.
  • Produce accurate, neat and thorough work.
  • Prioritize and plan work time efficiently; effectively managing competing demands and dealing with changes, delays and unexpected events.
  • Maintain a good understand and consistently comply with agency policies and procedures (including time and attendance, dress code, etc.).
  • Support a positive work atmosphere by behaving and communicating in a manner that allows the employee to get along with clients, students, coworkers and supervisors.
  • Communicate (in writing as well as orally) clearly and concisely.
  • Illustrate flexibility and adaptability in role and function when circumstances warrant; (demonstrate openness to assist in needed responsibilities in evolving situations).
  • Maintain appropriate boundaries, ethics and professionalism; comply with agency, OASAS and other professional code(s) of ethics; seeking supervision when challenges arise.


Who You Are:

Registered Nurse with a graduate education in nursing.

? Critical thinker

? Ability to cope well with stress, since the position includes direct involvement with clients suffering emergencies and other pressures

? Previous experience, working as a Nurse Practitioner, working in a related treatment, therapeutic community, community residence, and/or mental health setting working directly with special populations required ? Minimum of 2 years of experience working as a Nurse Practitioner in a mental health and/or substance abuse treatment or related setting;

? Ability to assess health related needs of the clients by monitoring ongoing health and wellbeing

? Able to provide and/or coordinate health services and health-related activities for clients. ? Availability to provide on call nursing for times when we do not have a nurse on site and serve as the health care coordinator.
