Job Description

MAJOR RESPONSIBILITIES: Facilitates all facets of youth Case Management under the direction of the Family Engagement Specialist. Serve as a member of the Youth Treatment Team (coordinating/facilitating meetings). Maintain Department of Children and Families Group Home requirements; and any other contractual agency requirements.

JOB RESPONSIBILITIES/DUTIES INCLUDE BUT ARE NOT LIMITED TO:

  • Responsible for client supervision and monitoring, medication monitoring, and transportation
  • Responsible for scheduling sessions with youth to establish, track, and complete case management goals to be reported in treatment meetings; and shared with Group Home Administrative Team
  • Responsible for the completion and implementation of life skills assessments and learning plans
  • Responsible for planning and facilitating quarterly Master Treatment Plan (MTP) meetings
  • Responsible for submitting quarterly progress reports and 30-day Case Action Plans
  • Responsible for assisting with enrolling youth in school; all school-based meetings
  • Responsible for assisting in the recruitment and service implementation of community volunteers
  • Responsible for linking youth to community services (recreation, volunteer, employment)
  • Responsible for assisting youth with career exploration, employment attainment, and retention
  • Facilitate/coordinate educational groups and workshops (Life Skills/Psychoeducational)
  • Maintenance of all electronic and hardcopy records
  • Community networking and committee involvement
  • Participate in agency staff meetings, events, agency/community workshops, and trainings
  • Responsible for assigned on-call duties as arranged by the Director of Group Home Services
  • Responsible for learning and carrying out the mission, policies, and procedures of LIFETIES, INC.
  • Responsible for being a positive representative of LifeTies, Inc. in your appearance, actions, and demeanor at all times
  • Responsible for petty cash – use and documentation
  • Responsible for planning/attending recreational activities for youth
  • Other duties as assigned

Qualifications

Spanish speaking candidates highly preferred. The Case Manager must have a Bachelor’s Degree (social work, psychology, or counseling). Three years’ experience working with youth and families is required along with experience working with the population served (specifically with pregnant and parenting youth and youth with chronic illnesses such as asthma or diabetes). Experience working in a Children’s System of Care residential placement preferred. The Case Manager must have a valid driver’s license, pass a criminal background check, and all other agency requirements.
