Job Description

• Assess patient’s nutritional status based on past medical history, diagnosis, biochemical and anthropometric data to initiate nutritional assessments, interventions and develop care plans for both sub-acute and long term care units.
• Complete nutrition care plans, progress notes and discharge plans in a timely manner
• Document all initial, annual, quarterly assessments and MDS forms to meet regulatory agency guidelines.
• Perform meal rounds, provide preferences via Geri-menu software
• Participate in interdisciplinary care plan meetings

Education and Experience:

• Registered Dietitian preferred or RD-eligible with the American Dietetic Association.
• Must have 1 year of nursing home experience
• EMR documentation, Geri menu a plus
