Job Description

Position Summary:

Responsible for the management of Nurse Clinical Coordinators involved in communication and caring for patients receiving specialty infusions as well as patients receiving specialty pharmacy therapies.

Essential Functions:

  • Develop and manage processes for coordination of care by the Nurse Clinical Coordinators for specialty infusion patients, specialty patients and clinical programs.
  • Obtain, enter and update referral information in EMR.
  • Discuss insurance limits and parameters of policy with patients prior to beginning services.
  • Act as a liaison between home health agencies, home infusion center and physicians to coordinate admission to services and discharge from service, plan for patient care and establish Plans of Treatment.
  • Oversee the referral process and maintenance of EMR.
  • Coordinate therapeutic monitoring pertinent to therapy with home health care agency, laboratory and specialty infusion center pharmacy.
  • Coordinate pharmacy schedule with delivery of drugs and supplies. Verify disposable supplies are packed and are appropriate for patient needs.
  • Coordinate with physician home health care agency, laboratory and specialty pharmacy for injectable self-administration teaching, lab coordination and shipment arrangements.
  • Coordinate patient care at refill establishing current response to therapy and ADR/Adherence occurrences, forwarding all clinical calls to appropriate clinician, under the supervision of the pharmacist
  • Provide in-services on performance of required infusion related tasks to home health agency personnel and others as necessary.
  • Communicate, be a resource and establish guidelines internally with the clinical and management Staff and externally with the home health care agencies for IV therapy protocols and product use.
  • Responsible for management of Nurse Coordinators including but not limited to new employee orientations, 90-day reviews, annual employee reviews, training, competency assessments, employee discipline, work schedules and duties.
  • Responsible for monitoring the productivity metrics of managed staff, to include but not limited to phone logs, emails, EMR data (progress note reports, queue workloads and flow etc.). Developing new metrics as needed to allow for ongoing productivity measurements.
  • Responsible for all activities and duties of Nurse Clinical Coordinators include but not limited to appropriate communications via progress notes, arranging nursing, patient communications, health care professional communications, completing scheduling board, as well as scheduling shipments via patient contact and communication in computer system, e-mail system and verbally.
  • Participate in center QA&I to ensure achievement of excellence.
  • Responsible for complying with all Policies and Procedures and Pharmacy and Nursing laws and regulations.
  • Quality Committee Member

  • Participate and perform other duties as assigned by leadership

Competency:

  • Knowledge of standard medical terminology

Qualifications

Education/Experience Requirements:

  • Registered Nurse with an active license in the state employed.
  • At least 5 years nursing experience required
  • At least 5 years of management experience required
  • Prior IV therapy or specialty pharmacy experience preferred
  • Participate in any clinical program competencies as developed within company Policy and Procedures
  • Meets State Board of Nursing, Health Rehabilitative Services and accreditation agencies requirements
