JOB SUMMARY

Within an ambulatory clinic practice setting, in the context of a clinical care team and a Patient Centered Medical Home practice, provide full spectrum ambulatory care with a focus on acute and “urgent” care, and to a lesser extent, chronic disease management, and wellness and preventative care for adult and pediatric patients. The Advanced Practice Provider (APP) evaluates, assesses, diagnoses, prescribes and educates patients regarding their particular illness or chronic disease. The position provides high quality primary care to all patients, evaluates and treats acute and chronic problems for the patient population, completes histories, thorough assessments and physical examinations on all patients with accurate and complete documentation reflected in medical records and collaborates and communicates with other departments and colleagues, such as the pharmacy to ensure optimal patient care. The practice is a teaching practice with learners of different professions and disciplines with mechanisms in place for colleagues to ask one another questions.

POSITION RESPONSIBILITIES/DUTIES

Full Time Position with the following distribution/allocation of workload:

  • Clinical/Direct Patient Care (.50 cFTE - onsite)

In person acute care/same day access appointments up to 11 visits/session (typical average is 9).

  • Provide quality, accessible, cost-effective primary care for patients served by CU Medicine community practices.
  • Provide direct patient care for acute care patients who require/request same-day appointments.
  • Collaborate as appropriate with medical staff in the care of complex patients.
  • Interact with other health professionals to create guidelines and coordinate patient care.
  • Collaborate with other team members in chronic disease management and patient care management.
  • No call pool duties.
  • Provide follow-up care to patients who were hospitalized or had Emergency Department visits
  • Provider will not maintain a continuity patient panel.
  • Support and collaborate with medical, nursing and office staff.
  • Preferred administrative day is Wednesday.
  • Clinical Administration (.50 FTE)

Provide Epic in-basket management support to other clinic’s providers as assigned, triage doc pools, and complete inbox issues from patients recently seen by provider. Also, serve as a point of contact for staff (RNs, front desk) for acute patient issues not requiring appointment to address.

  • Meet with Medical Director/clinic director monthly at first to discuss needs and schedule.
  • New tasks appear continuously throughout the day, plan to check in-basket continually during clinic hours. Triage boxes will need to be at addressed at the end of AM and PM clinic sessions.
  • Refill medications, review chart in detail, check PDMP for controlled substances. Task to MA pools if patient needs to schedule an appointment.
  • Review any "flagged" labs and notify patient of results.
  • Answer MHC messages and pt calls in assigned provider in-baskets and take care of the message completely, coordinate with the PCP if they are in clinic if needed, or leave for PCP to address if unable to address without personal knowledge of patient. Protocols to be developed.
  • Place referrals if appropriate and update care gaps when possible.
  • Attempt to close the loop in as many tasks as possible and document work done on any tasks worked on, but not completed (e.g. messages left, outreach attempted).
  • Liaise with RN Triage, RN Care Manager and/or RN Practice Manager for messages requiring next level clinical decision support when PCP is not available.
  • Other tasks such as participation in QI projects related to improving QI metrics.
  • Administrative/Professional Development

As part of being a faculty member in an academic environment, participate in educational and research activities as opportunities present themselves and/or other duties as assigned by clinical leadership.

  • Maintain eligibility for School of Medicine Faculty.
  • Keep abreast of the latest developments in field, both academically and through applied skill.
  • Attend and present at conferences, meetings and trainings in order to maintain certification and enhance professional skills; participates in faculty development activities.
  • Accept other responsibilities assigned by the Medical Director.

Salary and Benefits:

The salary range (or hiring range) for this position has been established at 92,547 – 136,875.

The salary of the finalist(s) selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training.

Qualifications


MINIMUM QUALIFICATIONS
:

Applicants must meet minimum qualifications at the time of hire.

  • Masters/Doctor of Nursing practice degree or Physician’s Assistant degree.
  • Licensed as a Registered Nurse in the State of Colorado, if not PA.
  • Licensed/registered as a Family Nurse Practitioner with the State of Colorado, if not PA.
  • Completion of required 1,000 hours (provisional prescription authority/mentorship) or other minimum as defined by State of Colorado in order to obtain full prescriptive authority and DEA registration.
  • Medicare/Medicaid credentials.
  • National certification by ANCC or AANP as family nurse practitioner, if not PA.
  • Knowledge and skills necessary to provide care appropriate to the age of the patients served; the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs, and to provide the care needed as described in the office’s policies and procedures.

PREFERRED QUALIFICATIONS

  • Minimum 3+ years (full time) of FNP or PA experience working in a Patient Centered Medical Home/Ambulatory Family Medicine practice preferred and comfortable charting in Epic.
  • X-DEA waiver and experience with gender affirming hormone prescribing encouraged
